Specialist Care for Complex and Unique Needs
Some individuals need more than standard personal care. Our specialist care service provides expert, person-centred support for a wide range of complex conditions — delivered by trained, experienced carers who understand the full picture.

About This Service
What Is Specialist Care?
Specialist care provides support for adults with complex, high-acuity, or multi-faceted needs that go beyond standard domiciliary care. Unlike general home care, specialist care requires carers with additional training, deeper condition-specific knowledge, and the professional resilience to work effectively with individuals who may have challenging presentations, dual diagnoses, or highly variable support needs.
At Premier23, our specialist carers hold verified training and demonstrable experience across a wide range of conditions — including mental health, learning disabilities, acquired brain injury, physical and sensory impairments, neurological conditions, autism spectrum disorder, eating disorders, and substance misuse. Every placement is individually matched: we identify carers not only by qualification, but by personal suitability, lived understanding, and the right professional temperament for each person we support.
We work closely with NHS community teams, local authority social care commissioners, independent case managers, and families to deliver fully integrated, evidence-informed support. Whether an individual is transitioning from inpatient services, holding a direct payment, or being referred by a statutory team, we have the infrastructure, clinical relationships, and expertise to respond.
“We do not place carers without the relevant knowledge, experience, and personal suitability for the individual's specific condition. Every specialist placement is earned.”

Our Approach
How We Deliver Specialist Care
Complex care demands a higher standard of rigour at every stage — from the first conversation to the daily delivery of support. These are the four principles that define how we work.
Thorough Assessment
Before care begins, we invest time in understanding the full picture — the individual's history, strengths, goals, risk factors, and any professional involvement. Our assessments are collaborative, not transactional.
Expert Carer Matching
We match carers by condition-specific training, lived professional experience, and personal suitability. A carer who is technically qualified but temperamentally mismatched will not be placed. Every specialist match is earned.
Multi-Agency Collaboration
We work within your existing professional network — community mental health teams, LD services, social workers, district nurses, and case managers — communicating clearly and acting as a reliable, coordinated partner.
Adaptive, Evolving Care
Complex needs do not stay still. Our care plans are reviewed regularly and updated whenever circumstances change — ensuring that the level of support is always calibrated to the individual's current situation.
