Physical Disability Support That Puts You in Control
Physical disability support is skilled, professional assistance provided to adults whose physical condition — whether from birth, injury, illness, or progressive disease — affects their ability to manage daily tasks independently. It encompasses everything from personal care and manual handling, to equipment use, community access, and clinical coordination.
At Premier23 Care Services, our approach to physical disability support is built on a rights-based foundation. We start from the position that every adult with a physical disability has the right to live independently, to make their own decisions, to participate fully in their community, and to receive care that respects and promotes those rights at every step. Our role is not to do things for people — it is to support them to do what they choose.
Our carers are trained in specialist physical disability care competencies — including safe manual handling, correct use of mobility equipment, skin integrity and pressure care, and adapted personal care techniques. This is not generic home care with a disability label attached. It is specialist support delivered by skilled, person-centred professionals.

Parkinson's Disease
Patient, skilled support for individuals living with Parkinson's — managing medication timing, meal assistance, personal care, communication, and mobility with understanding and calm.
Spinal Cord Injuries
Specialist support for those with paraplegia, tetraplegia, or incomplete spinal cord injuries — including catheter care, pressure area management, and complex personal care.
Stroke Aftercare
Skilled physical rehabilitation support following stroke — working alongside NHS physiotherapy and occupational therapy teams to support recovery and long-term function.
Motor Neurone Disease (MND)

How It Works
Four Simple Steps
Initial Conversation
Call 01256 338456 or complete our form. We discuss the individual's condition, current level of support, and what they would like their support arrangement to achieve.
Comprehensive Needs Assessment
A senior care staff member conducts a thorough assessment — covering physical needs, equipment requirements, risk assessment, communication preferences, and the individual's own goals for independence and daily life.
Clinical Team Coordination
Where relevant, we liaise with the individual's OT, physiotherapist, specialist nurse, or GP to ensure our support plan is consistent with their clinical guidance and equipment prescriptions.
Support Begins and Evolves
Your care starts on the agreed schedule. We review the plan regularly — both proactively and in response to any change in condition — ensuring support always reflects current needs and goals.
Funding Your Care
How Physical Disability Support Can Be Funded
Physical disability support can be funded in several ways. Our care managers will help you understand your options and navigate any assessments or applications required.
Direct Payments
If you have been assessed as eligible by your local authority, you may receive Direct Payments to purchase your own care. Premier23 Care Services can be engaged directly as your provider, giving you full control over how your support is delivered.
NHS Continuing Healthcare (CHC)
Individuals with a primary health need arising from a disability, accident, or illness may qualify for NHS-funded care at home. Our care managers can support you through the CHC assessment process and work with ICB commissioning teams.
Personal Health Budgets (PHB)
A Personal Health Budget is an amount of NHS money allocated to support your health and wellbeing needs. PHBs can be used to purchase personalised care from Premier23, giving you greater choice and flexibility over your support.
