What Is Community Mental Health Support at Home?
Community mental health support is non-clinical, practical, and emotional support provided to adults living with mental health conditions in their own home or community. It is distinct from NHS mental health treatment — it does not replace therapy, psychiatry, or clinical intervention. Instead, it provides the consistent, day-to-day support that helps people implement what their clinical team recommends, maintain their independence, and build a stable, fulfilling life.
Mental health conditions affect every area of daily life — from managing a routine and maintaining a home, to accessing the community, managing finances, taking medication, and maintaining relationships. Community support addresses these practical realities in a non-judgemental, person-led way — meeting the individual where they are and supporting them to move toward where they want to be.
At Premier23 Care Services, our mental health support workers are trained in recovery-focused approaches, trauma-informed practice, and positive risk-taking. We do not manage people — we work alongside them. Our role is to be a consistent, trusted presence in the community — one that adapts to good days and difficult days alike, without judgement and without withdrawal.

Recovery-Focused Assessment
We conduct a recovery-focused assessment — exploring daily functioning, goals, community access, risk, and what the individual identifies as their own priorities for support and growth.
Support Worker Matched for Trust and Compatibility
We take matching particularly seriously for mental health support. A good relationship is the foundation of effective community mental health care — we invest time in finding the right person.
Consistent Support With Clinical Coordination
Support begins on the agreed schedule. We liaise with the individual's community mental health team, GP, or other clinical professionals — and we review the support plan regularly with the individual.
Working With Your NHS Team
Community Support That Works Alongside Your Clinical Care
Our mental health support service is not a replacement for NHS mental health treatment, therapy, or psychiatric care. It is designed to complement clinical care — providing the practical, consistent, community-based support that NHS teams cannot always offer in the frequency needed.
We communicate with community mental health teams, GPs, and crisis services — both to receive guidance on how to support each individual appropriately, and to flag any concerns about a person's mental state or safety. This joined-up approach is essential for effective community mental health care.
