Safe, Accurate Medication Support in the Comfort of Home
Managing multiple medications correctly is one of the most common challenges facing individuals at home — and one of the most important. Our CQC-registered carers provide trained, accurate, and dignified medication support, ensuring every dose is taken correctly, safely, and on time.
From a simple daily reminder to full medication administration and GP liaison, we provide the level of support that is right for your loved one — with precision, care, and a complete written record every time.

Why This Matters
Why Getting Medication Right Matters
Medication errors are one of the leading causes of avoidable harm in home care settings. Research estimates that medication errors contribute to tens of thousands of preventable hospital admissions per year in England, and that many of the most serious errors occur at home — where no clinical oversight exists without professional support.
For older adults managing multiple long-term conditions, polypharmacy — the simultaneous use of multiple medications — significantly increases complexity and risk. Incorrect doses, missed doses, wrong timing, and confusion between medications are all real and preventable risks when the right support is in place.
At Premier23 Care Services, our carers are trained to manage medication support with precision, vigilance, and care. We treat medication management as a safety-critical responsibility — requiring thorough training, clear records, and regular communication with GPs, pharmacists, and families.
“The right medication, at the right dose, at the right time. For someone at home managing complex prescriptions, this is not a small thing.”

Our Medication Services
The Different Levels of Medication Support We Provide
Medication support is not one-size-fits-all. The level of involvement depends on the individual's needs, capacity, and prescription complexity.
Medication Prompting
For individuals capable of taking their own medication but needing reminders. The carer prompts at the correct time and records that medication has been taken.
Suitable for
Mild memory difficulties, those prone to forgetting, physically capable individuals needing oversight.
Medication Administration
For those who cannot reliably manage medications. The trained carer prepares doses, manages dosette boxes, and records each administration accurately.
Suitable for
Moderate to severe memory loss, dementia, physical limitations, complex multi-drug regimens.
Full Medication Management
Comprehensive oversight — ordering, collection, storage, administration, record-keeping, and coordination with GPs and pharmacists. Includes side-effect monitoring and prescribing liaison.
Suitable for
Complex multi-drug prescriptions, those without a family carer, recently discharged from hospital.

Our Safety Standards
How We Keep Medication Support Safe
Comprehensive Medication Training
All carers providing medication support complete a thorough training programme — covering safe administration, record-keeping, recognising side effects, and handling refusals.
Written Medication Administration Records
Every administration is recorded — medication name, dose, time, and carer signature. Records available to family and clinical teams at any time.
CQC-Compliant Medication Policies
Our policies are written to CQC standards and reviewed regularly. Compliance is monitored through supervision, spot checks, and record review.
Prompt Communication of Concerns
Any change in condition, suspected side effect, missed dose, or prescribing concern is communicated promptly to family, GP, or pharmacist — with a clear record of action taken.
Regular Medication Reviews
We participate in and support regular medication reviews — flagging anything observed in day-to-day care that may be relevant to prescribing decisions.

Who We Support
Who Benefits From Medication Support at Home?
Medication support suits a wide range of individuals. It can form part of a wider care arrangement or stand as a dedicated service in its own right.
Those With Memory Difficulties or Dementia
Memory loss makes independent medication management unreliable. Consistent oversight ensures correct adherence every day.
Older Adults on Multiple Medications
Managing five or more medications simultaneously is cognitively demanding. Support reduces mental load and risk of errors.
Those Recently Discharged From Hospital
Post-discharge regimens are often new, complex, and frequently changed. Our support bridges the critical recovery period.
People With Physical Limitations
Opening blister packs or using inhalers can be physically difficult. We provide practical, dignified assistance.
Adults With Mental Health Conditions
Medication adherence is particularly important — and sometimes challenging — for adults with mental health conditions. Non-judgemental support.
Those Without a Family Carer Nearby
Professional coverage including prescription collection, GP liaison, and emergency escalation for those living alone.

Medication Assessment
We conduct a detailed medication review with the individual, their family, and — where appropriate — their GP or pharmacist. We identify the correct level of support and any risks.
Medication Care Plan Created
A clear written medication care plan is developed — detailing every medication, dose, timing, and the carer's specific responsibilities. Shared with family and prescribing team.
Ongoing Support With Regular Review
Medication support begins on the agreed schedule. Every administration is recorded. The plan is reviewed regularly and updated whenever prescriptions change.

Client Stories
What Our Clients Say
“My father is on eleven different medications across the day. Before Premier23, he was missing doses regularly — and we had no way of knowing. Within a week of their medication support starting, his GP noticed the improvement. The carer is meticulous — every dose recorded, every change communicated. For the first time in years I feel confident that his medications are being managed properly.”
