Discharge Briefing & Home Preparation
We liaise with the hospital discharge team or your family to gather a full briefing. We can also advise on preparing the home environment before arrival — equipment, safety, access.
Carer Present on Discharge Day
A trained, briefed carer is at the property when your loved one arrives home — or accompanies them from hospital if arranged. There is no gap in support.
Ongoing Support & Smooth Transition
We monitor and adjust the care plan as recovery progresses. When the time is right, we can transition seamlessly into a longer-term domiciliary or live-in care arrangement.

We Work Alongside NHS and Healthcare Teams
Premier23 does not operate in isolation. We work closely and cooperatively with NHS discharge coordinators, district nurses, occupational therapists, physiotherapists, and GPs to ensure that the transition from hospital to home is joined-up, safe, and well-communicated.
