About This Service
What Is End-of-Life Care at Home?
End-of-life care — sometimes called palliative care or terminal care — is support provided to someone who is approaching the end of their life. Its purpose is not to treat or cure, but to ensure that the individual is as comfortable, dignified, and supported as possible during this time.
For many people, being at home during this period is deeply important. It is where their memories live, where they feel most themselves, and where the people who matter most to them can be close. Research consistently shows that most people, when asked, would choose to spend their final time at home. At Premier23 Care Services, we are committed to honouring that wish wherever possible.
End-of-life care at home is a team effort. Our carers work closely alongside GPs, district nurses, hospice teams, and palliative care specialists — communicating clearly and acting promptly so that care is always appropriate, consistent, and fully joined up.

Premier23 Care Services does not work in isolation. End-of-life care at home is most effective when it is fully joined up — and we are experienced in coordinating closely with all the professionals involved in your loved one's care.
GPs and Palliative Consultants
We communicate promptly and clearly with your loved one's GP and any palliative consultants involved — ensuring care is always appropriate, medications are correct, and any changes in condition are acted upon without delay.
District Nurses and Community Nursing Teams
Our carers work alongside district nurses for clinical tasks such as wound care, catheter management, and syringe driver monitoring — ensuring skilled, continuous care between nursing visits.
